"Does truth ultimately prevail ? History tends to say yes in medical innovations Here are some truths about the sacroiliac joint (SIJ): 1. As the largest synovial joint in the axial skeleton, the SIJ has extensive cartilage endplates and a significant lubricant presence. 2. The practice of using a relatively small, intra-articular device for SIJ fusion overlooks the joint's large surface area and may explain the current lack of supporting radiologic evidence. 3. Arguing over who is technically qualified to perform SIJ fusion with implants ignores the ethical implications of inserting non-retrievable devices in patients. Sacrix, Medtronic, and SI-BONE have invented various techniques and titanium implants to transfix across the SIJ to address the fusion challenges. The Sacrix implant, for instance, features a complex multithreaded matrix design with RBM surface texturing and the technique was aimed at crossing from posteriorly to anteriorly using two implants separated from each other to increase the chance of immediate stabilization and eventual fusion.
