Epidemiology of Back Pain

The epidemiology of low back pain (LBP) highlights its significant impact on global health, affecting a vast number of individuals and being a major cause of disability worldwide. As of 2020, LBP affected approximately 619 million people globally, with projections estimating an increase to 843 million by 2050, primarily due to population growth and aging. LBP can occur at any age, with most individuals experiencing it at least once in their lifetime. It is more prevalent in women and typically increases with age, peaking around 50–55 years. Non-specific LBP, where a specific cause is not identifiable, accounts for about 90% of cases​​. A comprehensive review on the prevalence of LBP in the adult general population indicates its significant prevalence and impact. Point prevalence is approximately 12%, with a one-month prevalence of 23%, a one-year prevalence of 38%, and a lifetime prevalence of around 40%. The review also underscores the influence of various comorbid factors such as obesity, smoking, lack of exercise, and psychological disorders, which contribute to the risk and exacerbation of LBP. This condition not only affects individual well-being but also imposes a substantial economic burden due to healthcare costs and loss of productivity​​. Management and diagnosis of LBP involve a thorough patient history, physical examination, and, when necessary, diagnostic testing to rule out specific causes. Common causes include musculoskeletal issues, degenerative diseases, and, less commonly, infections or tumors. Initial management typically emphasizes conservative treatments such as physical therapy, lifestyle modifications, and pain management strategies. Diagnostic imaging and more invasive treatments are reserved for cases where specific serious conditions are suspected or if the pain persists despite conservative management​​.
